Shown to staff along with your allergens β a first name usually works best so whoever's helping you (a waiter, a teacher, a paramedic) knows who they're talking to. A nickname's fine too if you'd rather.

Shown on your card so staff can reach someone on your behalf β this is never a substitute for calling 999 in a real emergency. The relationship only ever shows inside the π¨ Emergency screen, so whoever's calling knows who they're reaching.

A short line only ever shown inside the π¨ Emergency screen β not the everyday card. "You've got this, call me if you're not sure" is plenty. Purely personal, never required.
The π¨ Emergency screen's "Send SOS" button drafts a text to the contact above in your own Messages app β you tap send yourself, so it only ever uses your own phone's texts. Add up to 2 more names/numbers here to keep them on record too β they'll show up in your Allergy Passport for anyone helping to contact manually, though only the contact above gets the pre-filled SOS text.
Add one short line to the SOS text (optional). It goes out after the emergency, the 999 instruction and your allergies β those always come first and can't be replaced. Use it for something only you'd know to say.
Up to 120 characters. Longer texts get split into more messages and cost more to send.
Real name, date of birth and home address help paramedics identify you and pass details to hospital. These never appear on the everyday card β only inside the π¨ Emergency screen, for use during an actual reaction.
